In 2016 the Health Conference of Baden-Württemberg (Landesgesundheitskonferenz BW) gave recommendation to institute centers for personalized medicine (ZPM). The three main aspects of these centers should be digital cross-linking of the centers, an efficient outreach strategy to include peripheral clinics and dissemination and participation approaches to inform and integrate the patients. These three aspects will be realized within this project by a web-based portal, providing independent information about personalized medicine, to support allocation of patient flows and facilitate data exchange between the centers in the background.

Especially the close communication to harmonize the personalized therapy decision processes in the molecular tumor boards (mtb) will further improve the quality and experience of pm by providing an evidence based database for prospective therapy decisions. After the initiation of the internal care associated functions, the participation of patients and their relatives will be implanted.

The PM-Portal will inform patients and all interested parties about the major topics of pm. High quality information and a better understanding of the challenges and opportunities of pm will help to avoid excessive expectations and prevent possible uncertainties of the patients. Information about the pm offers of each ZPM and about the access routes to the ZPM will be provided in an appropriate and target group-oriented way. The target group of the molecular tumor board is in the first project phase oncologists of the university hospitals and their patients with advanced, molecularly characterized tumors. In addition, the platform provides resident doctors and peripheral clinics access to the interdisciplinary panel of experts, thus providing patients with improved care.

Durch die Vernetzung der Datensätze zu personalisierten Therapien aus den Molekularen Tumor Boards können die Universitätskliniken in BW ihren Wissens- und Erfahrungsvorsprung weiter ausbauen und auf einer breiten Datenbasis evidenzbasiert Therapieempfehlungen treffen. Neben der Verbesserung der Versorgung wird das PM-Portal zudem Patienten und Angehörigen eine unabhängige und qualitativ gesicherte Informationsplattform bieten, die zu einem besseren Verständnis des komplexen Themas PM und damit einer angepassten Erwartungshaltung führen wird.

Das PM-Portal wird Patienten und alle Interessierten über das Themenfeld der PM und die Angebote sowie Zugangswege zu den ZPM sachgerecht und zielgruppenorientiert informieren. Die Zielgruppe des Molekularen Tumor Boards sind in der ersten Projektphase Onkologen der UK und deren Patienten mit fortgeschrittenen, molekular charakterisierten Tumoren. Darüber hinaus bietet die Plattform für niedergelassene Onkologen und periphere Kliniken in kooperativer Weise den Zugang zu dem interdisziplinären Expertengremium und somit ihren Patienten eine verbesserte Versorgung.
